        <p>Преэклампсия (ПЭ) и гестационный сахарный диабет (ГСД) нередко сочетаются. Каждое из этих заболеваний представляет собой акушерское осложнение, негативно влияющее на перинатальные исходы матери и новорожденного. Инициирующим фактором развития ПЭ является ишемия плаценты, в ответ на которую у беременной женщины возникают эндотелиальная дисфункция, окислительный стресс и воспаление, обусловливающие симптомы ПЭ. Гипергликемия и инсулинорезистентность, сопровождающие ГСД, способствуют развитию ишемии плаценты и усиливают эндотелиальную дисфункцию, окислительный стресс и воспаление, отягощая таким образом состояние женщины. Нарушение строения ворсинчатого дерева ведет к ишемии плаценты, способной инициировать ПЭ. Гипергликемия, сопровождающая ГСД, препятствует соответствующему развитию ворсинчатого дерева, что может вызывать плацентарную ишемию. Данный обзор литературы рассматривает сочетание патогенетических механизмов ПЭ и ГСД, лежащих в основе их клинических проявлений и строения плаценты.</p>

      <trans-abstract xml:lang="en">
        <p>Preeclampsia (PE) and gestational diabetes mellitus (GDM) are frequently combined, both diseases causing common pregnancy complications and adverse perinatal outcomes for a mother and newborn. Placental ischemia is the initial factor of PE development followed by endothelial dysfunction, oxidative stress, and inflammation, which determine PE symptoms. Hyperglycemia and insulin resistance accompanying GDM contribute to placental ischemia and increase PE symptoms, thereby exacerbating maternal health. Abnormal development causes placental malperfusion and leads to preeclampsia. Hyperglycemia accompanying GDM prevents the appropriate evolution of villi, which can trigger placental ischemia. This review describes the pathogenesis of PE and GDM and focuses on their clinical features and placental pathology.</p>
